Visit Fee: $300
I am an out-of-network provider, which means I do not contract directly with insurance companies. You will be responsible for paying the full cost of services at the time of your appointment, and I will provide you with a superbill. I encourage you to contact your insurance provider to understand your coverage and any reimbursement options
Card on File: Before your first visit, you will be asked for a credit card to reserve your appointment, your card will not be charged unless you do not come to your appointment and do not cancel your appointment at least 24 hours in advance.
Direct Payment: After your visit, you will be asked to pay the fee using Zelle, Venmo, or the card on file. I will provide you with a superbill that can be used to submit a claim to your insurance.
If your insurance plan includes out-of-network coverage, you may be eligible for partial reimbursement of your medical expenses. Here’s what you need to know:
Verify Coverage: Contact your insurance provider to understand your out-of-network benefits, including deductibles, co-insurance, and any coverage limits.
Claim Submission: You will be responsible for submitting claims to your insurance company. The superbill will contain all necessary information for your claim.
Reimbursement: Any reimbursement will be sent directly to you from your insurance company. The amount can vary based on your specific plan and the services provided.
Deductibles: Be aware that out-of-network deductibles are often higher than in-network deductibles and may need to be met before any reimbursement is provided.
Payments made to my practice qualify for FSA and HSA reimbursement. I’ll provide appropriate documentation for your records.
